[Sarkari-Naukri] Regarding UPSC Recruitment Advertisement No. 19/2009

Hello Friends,

i want to apply in Assistant Programmer required by CBI which was posted in UPSC Recruitment Advertisement  No. 19/2009.
It is the first time i'm applying UPSC recruitment. So I am having problem with Application Form which are

1. There are 2 type of format - i don't know which one to follow
2. confusion in advertise No. and Item No (Ref.No.F.1/186/2009-R-II) - I don't know which number to consider Advertise Number and which to Item Number
3.Year  - Current Year ????
4.Where exactly application need to be sent??? (They haven't mention anywhere in advertisement)
